Diagnostika naudojant elektrokardiografijos (EKG) Holtero stebėjimą yra svarbus metodas šiuolaikinėje medicinoje, nes tai leidžia aptikti įvairias širdies ritmo ir laidumo anomalijas, kurios gali sukelti rimtų ligų ir mirtį.Tyrimo problema - PQRST kompleksų ir segmentų Holtero EKG įrašuose anotavimo algoritmų analizė ir įgyvendinimas. Tai taip pat apima EKG anotacijos grafinės vartotojo sąsajos (GVS) įgyvendinimo metodų analizę su integruotu įgyvendintu algoritmu.Tyrimas sutelktas į metodus, kaip sumažinti gydytojo laiko įtraukimą anotuojant Holtero EKG įrašus.Tyrimo tikslas yra nustatyti efektyviausius metodus, kaip įgyvendinti pusiau automatizuotą Holtero EKG įrašų anotaciją su tikslu sukurti didelį anotuotų EKG įrašų duomenų rinkinį ir sukurti automatinį patologijų aptikimo algoritmą be gydytojo įsikišimo.Šiame tyrime buvo pasiūlyta EKG įrašų anotacijos sistema su daugelio žymų palaikymu (Holtero stebėjimui - iki 500 000 žymų kiekvienai iš 3 EKG vedimų) su įgyvendintu PQRST kompleksinių bangų intervalų automatinės anotacijos algoritmu. Pirmą kartą EKG bangavimo anotavimo užduotyje buvo sukurtas algoritmas, skirtas kiekvienos P, Q, R, S, T bangos intervalų anotavimui, o ne tik jų viršūnių pozicijoms.Buvo nustatyta algoritmo tikslumo vertinimo metodika, pasiekiant 0.91 intervalo anotacijos tikslumą. Šis algoritmas atveria galimybę ateityje sukurti mašininio mokymosi klasifikacijos modelį širdies ritmo patologijoms, paremtą ne tik pačiu signalu, bet ir bangavimo intervalais.Be to, dėka įgyvendinto sprendimo, savanorių gydytojų komanda jau naudoja įgyvendintą anotacijos sistemą, anotuodama duomenų rinkinį būsimam klasifikatoriui su unikaliu žymų rinkiniu, įskaitant patologijų žymes ir gydytojo koreguotus bangų intervalus. Šis tyrimas sudarytas iš 52 puslapių, 21 paveikslėlio, 19 formuliučių, 1 lentelės.
The diagnosis using Holter monitoring of electrocardiography (ECG) is an important method in modern medicine as it allows for the detection of various abnormalities in heart rhythm and conduction.The research problem - the analysis and implementation of algorithms for annotating the PQRST complexes and segments in Holter ECG recordings. It also involves the analysis of methods for implementing a graphical user interface (GUI) for ECG annotation with integration of the implemented algorithm.The research focuses on methods to reduce the time involvement of a physician in annotating Holter ECG recordings. The objective of the study is to determine the most effective methods for implementing semi-automatic annotation of Holter ECG recordings with the aim of creating a large dataset of annotated ECG recordings and developing an automated algorithm for pathology detection without the need for physician involvement.In this study it has been proposed an ECG recording annotation system with support for many labels (for Holter monitoring up to 500,000 labels for each of the 3 ECG leads) with the implemented algorithm for automatic annotation of PQRST complex waves intervals. For the first time in the task of ECG waveform annotation, an algorithm was developed specifically for annotating the intervals of each of the P, Q, R, S, T waves, rather than just the positions of their peaks.A methodology for evaluating the accuracy of the algorithm was established, achieving an interval annotation accuracy of 0.91. This algorithm opens up the possibility for future development of a machine learning classification model for cardiac rhythm pathologies based not only on the signal itself but also on the intervals of the waveforms.Furthermore, thanks to the implemented solution, a team of volunteer physicians is already using the implemented annotation system to annotate a dataset for the future classifier with a unique set of labels, including pathology labels and physician-corrected wave intervals.
